Singapore’s Healthier SG Program: How Women Can Use It to Prevent Disease

On 5 July 2023, Singapore launched Healthier SG; the most significant reform of its primary healthcare system in decades. The program represents a deliberate national shift from reactive, illness-based care to proactive, prevention-based care: every eligible Singapore Citizen and Permanent Resident aged 40 and above is invited to enrol with a family doctor, receive a personalised health plan, and access subsidised health screenings that were previously not systematically available at this level.

By August 2024, close to one million residents, approximately 40% of the eligible population, had enrolled. A regression discontinuity study from Duke-NUS Medical School found that Healthier SG enrolment raised health screening uptake by 33.5 percentage points among newly eligible residents, confirming that the programme is meaningfully changing the preventive health behaviour of Singapore’s population.

For Indian women in Singapore, Healthier SG is not just a government program to be aware of, it is a direct, subsidised pathway to the metabolic screenings (fasting insulin, HbA1c, lipids, blood pressure) that this community needs most urgently, given South Asian women’s disproportionate risk of type 2 diabetes, hypertension, and metabolic syndrome at lower BMIs and younger ages than other ethnic groups.

What Healthier SG Actually Offers You

1. A Named Family Doctor

You choose and enrol with a participating GP clinic or polyclinic as your Healthier SG healthcare provider. This becomes your longitudinal primary care relationship, the doctor who tracks your health over time, not just treats you when you are sick. For Indian women who have been managing multiple disconnected GP visits for different concerns (thyroid here, iron there, gynaecology elsewhere), having a single family doctor who sees the complete picture is a meaningful clinical upgrade.

2. A Personalised Health Plan

Your enrolled family doctor creates a health plan based on your age, medical history, risk factors, and lifestyle, including recommended screenings, vaccinations, and lifestyle goals. This is documented digitally and accessible through the HealthHub app.

3. Subsidised Annual Health Screenings

Enrolled residents receive subsidised access to annual health screenings through the Screen for Life program, including:

  • Fasting blood glucose and HbA1c (diabetes and prediabetes)
  • Lipid profile (total cholesterol, LDL, HDL, triglycerides)
  • Blood pressure
  • BMI and waist circumference
  • Cervical cancer screening (Pap smear or HPV test, as covered in SG13)
  • Colorectal cancer screening (FIT test for those above 50)
  • Breast cancer screening (mammogram for women 40–69)

4. Subsidised Chronic Disease Management

From February 2024, the Healthier SG Chronic Tier allows patients with hypertension, diabetes, hyperlipidaemia, and (from January 2025) gout, chronic kidney disease, and heart disease to receive subsidised chronic medications at private GP clinics at rates comparable to polyclinics, expanding access beyond just polyclinics.


How to Enrol — Step by Step

Step Action
Step 1 Download the HealthHub app (iOS or Android) and log in using Singpass
Step 2 Go to the Healthier SG section and search for participating GP clinics or polyclinics near you
Step 3 Select your preferred clinic and confirm enrolment. You can change your enrolled clinic once per year if needed
Step 4 Book your first Healthier SG consultation with your enrolled clinic through HealthHub or by calling the clinic directly
Step 5 At your first visit, complete your health plan with your doctor, including which screenings are due and your health goals for the year

Healthier SG is currently available to Singapore Citizens and Permanent Residents aged 40 and above. If you received an SMS from MOH inviting you to enrol, act on it; the invitation is age-specific and signals you are now eligible.


What Indian Women Should Specifically Request at Their Healthier SG Consultation

The standard Screen for Life panel is an excellent starting point, but for Indian women with the thin-fat phenotype, standard metabolic screenings may underestimate risk. Here is what to ask your Healthier SG doctor to add, and why:

Request 1: Fasting Insulin and HOMA-IR

The standard Healthier SG metabolic panel includes fasting blood glucose and HbA1c, both of which only become abnormal after insulin resistance has been present for years. Fasting insulin, from which HOMA-IR (homeostatic model assessment of insulin resistance) can be calculated, detects insulin resistance a decade earlier. A HOMA-IR above 1.9 indicates insulin resistance even with normal glucose. For Indian women at higher metabolic risk, this test provides the earliest actionable warning. It may need to be specifically requested as it is not always part of the standard panel.

Request 2: Waist Circumference (Not Just BMI)

Singapore’s Healthier SG health plan includes BMI, but for South Asian women, waist circumference above 80 cm is a more sensitive indicator of visceral fat and metabolic risk than BMI alone. Ask your doctor to measure waist circumference and record it in your health plan, then track it annually.

Request 3: Vitamin D (25-OH)

Near-universal Vitamin D deficiency in Indian Singapore women is documented, yet Vitamin D is not part of the standard Screen for Life panel. Given its documented impact on insulin sensitivity (the PREVENT-WIN trial in Asian Indian women with prediabetes), thyroid function, bone health, and immune function, it is worth requesting specifically. Available at all polyclinics as an add-on test.

Request 4: Full Thyroid Panel

TSH is not part of the standard Healthier SG screen for most women, yet thyroid dysfunction is one of the most common endocrine disorders in Indian women and causes fatigue, weight gain, and mood changes that are frequently attributed to other causes. If you are symptomatic (fatigue, cold intolerance, hair loss, weight gain despite diet), request TSH and FT4 specifically at your Healthier SG consultation.

Request 5: Ferritin (Not Just Haemoglobin)

Haemoglobin only falls when iron deficiency is well-advanced. Ferritin, the stored iron marker, becomes abnormal years earlier and is the most sensitive early indicator. Request ferritin alongside the standard complete blood count, particularly if you experience fatigue, hair loss, or heavy periods.


Using Your Healthier SG Results: The Nutritional Response

Frequently Asked Questions

I am 38 and not yet eligible for Healthier SG. What can I do now?

The Screen for Life subsidised screening programme is available for certain screenings from age 25 (cervical cancer) and for diabetes screening at specific intervals for those with risk factors from a younger age. Even without Healthier SG enrolment, you can access subsidised blood glucose, blood pressure, and cervical screening at polyclinics through Screen for Life. Additionally, the Grow Well SG programme for your children (SG07) is available regardless of your own age. Use HealthHub to check what you are eligible for now, and set a calendar reminder for your 40th birthday to enrol in Healthier SG immediately upon eligibility.

My Healthier SG doctor gave me a three-minute consultation and sent me home with some leaflets. Is this normal?

The early implementation of Healthier SG has faced well-documented challenges; GP time constraints under the new model were specifically identified in a qualitative study of primary care physicians implementing HSG. If your Healthier SG consultation did not address your specific risk factors in depth, you have options: request a longer dedicated health plan appointment, ask specifically for the additional tests listed above, and supplement your GP’s preventive care with specialist input (an endocrinologist for metabolic concerns, a dietitian for nutrition) when needed. Healthier SG provides the framework; you can use it as a floor, not a ceiling, for your preventive health investment.

Can I use Healthier SG at a private GP rather than a polyclinic?

Yes, over 900 private GP clinics have enrolled in the Healthier SG programme. You can choose a private GP clinic near your home or workplace as your Healthier SG provider. The subsidies and chronic disease medication access apply through these clinics as well, though out-of-pocket costs at private GPs may be higher than at polyclinics for the non-subsidised components of care.

I have already been diagnosed with diabetes. Is Healthier SG still useful for me?

Absolutely, in fact, it is particularly valuable. Healthier SG’s Chronic Tier (from February 2024) specifically supports patients with diagnosed diabetes through subsidised chronic medications and structured chronic disease management. Your enrolled family doctor coordinates your care, monitors your HbA1c trajectory, and connects you to the appropriate specialist referrals through the Healthier SG network.
